Zygomatic Implant: The Last Frontier for Those Who Lack Jawbone

There is a profile of patients who, for decades, faced one of the greatest challenges in dentistry: those who lost so much bone in the upper jaw that no conventional implant could be anchored. For these people, the only options left were unstable dentures or huge reconstruction surgeries. Today, however, there is a technique that has completely changed this scenario: the zygomatic implant.


What is the Zygomatic Implant?


The zygomatic implant is a special type of implant, much longer than conventional ones, that is anchored in a very strong bone of the face: the zygomatic bone, commonly known as the cheekbone.


While a traditional implant measures between 8 and 15 millimeters, the zygomatic implant can reach 30, 40, or even 55 millimeters in length. How Does the Treatment Work?


Planning is the most important step. The dentist performs a detailed computed tomography scan and carefully studies the patient's anatomy, identifying the exact path that the implant will follow, avoiding delicate structures such as the maxillary sinus, nerves, and blood vessels.


Today, with digital planning and the creation of personalized surgical guides, the precision of the technique has increased considerably, making the procedure safer and more predictable.


The surgery is usually performed with local anesthesia combined with sedation, or in some cases with general anesthesia, depending on the complexity and comfort of the patient. The dentist positions the implants at a specific angle, starting from the area where the upper teeth used to be and reaching the zygomatic bone.


In most cases, two zygomatic implants are used, one on each side, usually combined with two or four conventional implants at the front of the mouth. This strategy, known as All-on-Four Zygomatic or Quad Zygoma (when four zygomatic implants are used), provides firm support for a complete fixed prosthesis.

Because they anchor in an extremely dense bone, zygomatic implants usually have excellent initial stability, allowing for the installation of a provisional prosthesis on the same day as the surgery or within 48 hours.

Are There Risks?


Like any complex surgical procedure, the zygomatic implant has risks that need to be understood. The anatomical region involved is delicate, close to the maxillary sinus, the eye orbit, and important facial structures.


When performed by highly qualified professionals with specific training and proven experience, the risks are minimized. The most common complications include sinusitis, which usually resolves with medication, and postoperative discomfort, controlled with pain relievers.

The recovery period varies for each patient. In the first few days, it is common to experience swelling in the face, especially in the cheekbone area and around the eyes. This swelling resolves in one to two weeks.


The prescribed medication controls discomfort, and the diet should be softer in the first weeks. Relative rest, careful hygiene, and close follow-up with the dentist are essential. Most patients resume light activities within a few days and their full routine in two to three weeks.


Some specific guidelines are important, such as avoiding blowing the nose forcefully in the first weeks, not smoking, and strictly following the professional's recommendations.


Long-Term Maintenance


The zygomatic implant, when well cared for, can last for decades. Maintenance follows the basic principles of any implant: rigorous oral hygiene, use of appropriate brushes and dental floss, and regular follow-up appointments.


Peri-implantitis, inflammation around the implant, is the main long-term concern. Therefore, regular visits to the dentist for professional cleaning and evaluation are essential to maintain the health of the implants and the prosthesis.


Who Can Perform This Technique?


The zygomatic implant requires highly specialized training. It is not enough to be an ordinary dentist or implantologist. The professional needs to have specific courses, practical training in specialized centers, and advanced surgical experience.


In addition, the clinic must have adequate infrastructure, including computed tomography, digital planning, specific surgical instruments, and safety protocols. In some cases, the procedure is performed in a hospital setting.


When looking for a professional, ask about their training, their specific experience with zygomatic implants, request to see previous cases, and openly discuss expectations and risks. Trust and transparency are fundamental.
